Reablement support at home

Norfolk First Support provides up to six weeks of support and reablement in your home. This is so you can get back to doing things for yourself, or work out what help you need in the future. It's aimed at people who are aged 18 years and above who:

  • Have spent time in hospital and/or
  • Are struggling in the community. For example, your physical or mental health is deteriorating.

A practitioner needs to identify if you would benefit from reablement services. If they think you will, they'll refer you. A practitioner might be a:

  • GP
  • Community nurse
  • Social work practitioner
  • Therapist

If you are in hospital, a practitioner can refer you while you're there.

If you're not in hospital but you think you need a referral, contact our adult social care team.

If you're eligible

If we can help you, support workers will start to make regular visits, starting on a date that works for you.

In the first day or two after the support workers have started visiting, a reablement assistant practitioner will arrange a visit. They'll agree how we will support you.

We will then work with you to create a personalised support plan with what's important to you.

The reablement approach helps you:

  • Retrain or regain skills to become more independent in your home
  • Build confidence so you can learn to manage after a period of illness
  • Manage long-term conditions

We don't charge for the reablement support service.

But most people don't need support for the whole six weeks and often return to being independent before that.

Sometimes we realise after a few weeks of working with you that you might benefit from a different service to reablement. If you need a different service either before or after the six weeks of reablement, support will not be free. Don't worry, we will be able to support you in getting the right help. We can also discuss any costs with you.

What happens when the First Support team visit you

When any of our team visits your home, they will always:

  • Show their identity cards when they visit you for the first time
  • Be dressed in the team uniform
  • Behave professionally and politely
  • Respect your privacy and dignity
  • Ensure you get prompt, appropriate support and tell you if there is a delay
  • With your agreement and if appropriate, make ongoing referrals to other support

We are currently looking for people to join our Norfolk First Response team. You'll support people to be as independent as possible and to live in their own homes for as long as they are able.

We have a variety of roles in different areas. So whether you'd like to help people in their homes or work in an office or a hospital, we could have the right role for you.
