Young person's needs assessment

The young person’s needs assessment, looks in detail at the young person’s social care needs.

It is used to decide if a young person age 16-25, meets the eligibility criteria for help from adult social care services.

If the young person is assessed as needing a service from adult social services, they will be offered a meeting with the joint visiting team.

The team will be able to help:

  • Complete a financial declaration form
  • Check the young person is receiving all the state benefits they are entitled to. They can helping them to claim these where necessary
  • Explain the charging policy of adult social care services 
  • Answer any questions
