Reablement assistant practitioner
Reablement Assistant Practitioners are the people who visit service users in their own homes to agree a plan of how NFS will support the person to be able to manage daily activities, such as their own personal care, meals and medication.
What you'll be doing
- Visiting people in their own homes to discuss with them what they would like to be able to do for themselves, given the right help. Following this discussion agree how many visits a day NFS will be carrying out
- Complete any relevant risk assessments for risks to the service users and/or support workers
- Arrange a review visit to check progress and agree what levels of care and support they may need after NFS services finish. This is managed by completing a Care Act Assessment
- Discuss with people what costs of service may be, if ongoing help is needed when NFS finish, and how this will be worked out
- Work with multi-disciplinary teams of other health and social care professionals to provide the best support to people and their families
- Using the departments case management system to receive referrals and record all relevant information
- Using a scheduling system to record and monitor support worker activity and plan home visits
