Reablement practitioner

Our reablement practitioners work in Norfolk First Support.

What you'll be doing

  • Line manage a team of up to 14 support workers
  • Visit people in their own homes to assess reablement goals and determine levels of functional independence
  • Assess levels of support needed after a period of reablement
  • Work with multi-disciplinary teams of other health and social care professionals to provide the best support to people and their families

About you

What you’ll need:

  • Time management skills
  • To be keen, passionate, helpful and able to show empathy
  • Good communication skills
  • Supervisory skills
  • A full driving licence and a car available for work
  • To be hard working and committed to supporting others
  • Problem solving skills
