What is My Care Record?

My Care Record is a partnership project to improve care by joining up health and care information.

Organisations may hold records about you separately, such as your:

  • GP
  • hospital
  • community health teams
  • mental health teams
  • social care teams (Norfolk County Council)

This means it can be difficult for us to work together to deliver the best care to you.

When information is joined-up, health and care professionals can provide faster, more effective care for you. Health and care organisations in our region are working together to implement a new sharing approach called My Care Record. Wherever possible, health and care professionals will be able to access your records from other services when it is needed for your care.
