Your social care practitioner can help you to make a claim as part of your care needs assessment or your review.
What is a care needs assessment
A care needs assessment is a meeting to talk about the support to need to live a healthy and fulfilling life. You can ask to have a review of your assessment if you think you need more support or different kinds of support.

How to fill in the form
1. Tell us how much money you spend on each of the things on the form.

2. Send us copies of receipts, bills or bank statements to show how much you spend.

3. We may ask for extra information if you are claiming for laundry, clothing, food or bedding.

4. It is really important you tell us about all the extra things you have to pay for because of your disability or illness. You will need to tell us what they are and how much they cost.
We may ask for extra information to support your claim. This may include a letter from your doctor.
If you need help filling in the form
If you need help filling in the form, call the DRE team and they can fill in the form for you. Or someone can come and help you fill it in at home.

Send your form to the DRE team. Make sure to include the receipts and bank statements we have asked for.

What the DRE team will do
The DRE team will look at your claim. They will decide if the costs you have claimed are disability related expenses. They will then write to you to explain their decision.
If your claim is successful
If your claim is successful, the amount you pay Norfolk County Council for your care will go down.
The amount you pay will be reduced by the amount of Disability Related Expenses allowance you are given.
