The Pitch Allocation Application form may not be accessible for some users.

The Pitch Allocation Application form cannot be saved and returned to later. Once started it must be completed. If you leave the form part way through before it is submitted all your information will be lost and you will need to start again.
Before you start we advise that you have access to all the relevant documents you will need to complete the form.
Essential documents to have to hand before you start: Proof of Identity and National Insurance Number (NI) for you and if relevant, your partner.
- This could be a passport, national identity card, immigration status document or other paperwork listed as acceptable in the Government's guide for right to rent checks. Provide either one document from List A, group 1, or 2 Documents from List A, Group 2.
- For NI: Recent benefits letter, or letter from HMRC
- Most recent 3 months bank statements
- Evidence of medical conditions, referral letter, full contact details of your GP - name, address, telephone number
- Name and address of schools your children attend
- Name and contact details of social worker, or any other support workers that are helping you and your family
- Details of employment
- Documentation relating to eviction, if relevant
- References - Contact details – name, address, telephone number, for all references, landlord of professionals who know you
