Primary care contracts
Lot 1a: NHS Health Check Programme - The Identification and Invitation of the Eligible Population
If you are a GP practice please contact the NHS Health Check team via phcontracts@norfolk.gov.uk with 'NHS Health Checks' in the title, to discuss identification of your eligible population and target setting/planning for the next five years.
If you are not a GP practice please can you contact the NHS Health Check team to discuss your planned provision of this service via phcontracts@norfolk.gov.uk with 'NHS Health Checks' as the subject.
Lot 1b: NHS Health Check Programme - Delivery of the NHS Health Check (including Standard Operating Procedures)
If you are an existing provider, please ensure that you deliver NHS Health Checks according to the standard operating procedures. Please provide the name and contact details for your clinical Cardiovascular disease (CVD) lead to the Health Check team via phcontracts@norfolk.gov.uk with 'NHS Health Checks' in the title.
New providers: please contact the NHS Health Check team to discuss training needs, planned delivery method for this service and provide us with the name and contact details for your clinical Cardiovascular disease (CVD) lead via phcontracts@norfolk.gov.uk with 'NHS Health Checks' in the title.
Lot 1c: NHS Health Check Programme - Community Records Update
If you are a GP please ensure that your secure email for NHS Health Check data transfer from community providers is up to date, and you have informed the NHS Health Check team of any changes via phcontracts@norfolk.gov.uk with 'NHS Health Checks' in the title .
If you not a GP practice please contact the NHS Health Check team to discuss your planned provision of this service via phcontracts@norfolk.gov.uk with 'NHS Health Checks' in the title.
Lot 2a: National Chlamydia Screening Programme, EHC, Pregnancy Testing
- Those new to delivering this service must contact the training provider - see the iCaSH website for details.
- Additional information for pharmacies is also available here
- Once new pharmacy providers have completed training please contact tracey.milligan@norfolk.gov.uk so that you can be accredited to access the relevant Pharm Outcomes templates. Please also log on to pharmoutcomes.org to view the services available to you. If you do not know your username, password or security words please send a message to the help desk via the Help tab. Further information about PharmOutcomes can be found here.
- For all pharmacy providers: Please note there will be a refresh of the sexual health pharm outcome templates to accommodate the new specification (from 1st April 2015), & notification of this will be given in a message via Pharm Outcomes
- New GP provider should contact tracey.milligan@norfolk.gov.uk with any queries.
- Other providers should email phcontracts@norfolk.gov.uk including the lot name in the title (Lot 2a National Chlamydia Screening Programme, EHC, Pregnancy Testing) to check compatibility of their reporting template and payment methods.
- New and current providers of these services must be competent in all relevant patient group directives (PGD's). Current PGDs are to be found here. Please note there are new PGD's from 31/12/14 for chlamydia treatment.
Lot 2b: Fitting and Management of Contraceptive Implants & IUDs/IUSs
- New GP providers should contact tracey.milligan@norfolk.gov.uk for inclusion onto the Commissioning Support Unit (CSU) reporting template
- Other providers should email tracey.milligan@norfolk.gov.uk including the lot name in the title 'Lot 2b Fitting and Management of Contraceptive Implants'
- Those new to delivering this service must contact the training provider: see the iCaSH website.
Lot 3a: Stop Smoking Service Provision
- Those new to delivering this service must contact the Specialist Stop Smoking Services: Smokefree Norfolk (0800 0854 113) for training, access to activity reporting systems and details of forms required.
- Once new pharmacy providers have completed training please contact alice.vickers@norfolk.gov.uk so that you can be accredited to access the relevant Pharm Outcomes templates. Please also log on to pharmoutcomes.org to view the services available to you. If you do not know your username, password or security words please send a message to the help desk via the Help tab.
- New and current providers of these services must be compliant with service protocols, standards and reporting procedures.
Lot 3b: Supply of Nicotine Replacement Therapy
- Those new to delivering this service must contact the Specialist Stop Smoking Services: Smokefree Norfolk (0800 0854 113) for training, access to activity reporting systems and details of forms required.
