Completing an Adult Social Care Online form

Completing a form

You can follow the on-screen instructions; we've provided further help below.

You must complete each page of the form before proceeding to the next. Please provide the information requested, when asked.  You must complete questions with an asterisk (*) for you to continue.

You do not have to complete a form all at once. You can start it, save it, and return to it later. You can do this by using the 'Save for later' button at the bottom of every page of the form.  

Personal details

You'll need to register for an account, or log in if you already have one. See Setting up an Adult Social Care Online login

Once you have logged in, you'll see your details. You cannot change the information in the grey boxes. You can add your telephone number if you have one. 

Who is this form for? 

This is where you need to provide the details of the person you are contacting us about.  You must answer the first question 'I am completing this form on behalf of' in the following way:

  • If you need support for yourself, choose 'Myself'
  • If you are telling us that somebody else needs support, choose 'Someone else'
  • If you are a professional, choose 'Someone else in a professional capacity'. If you're working with the person (either as a volunteer or paid worker) and do not have a personal relationship with them then we would consider you to be a professional. We have specific forms advice for professionals.

Complete the rest of the questions with the details of the person who needs support.  

Attachments

Some forms will provide an option for you to attach documents. You can upload them and then submit them alongside the form. Please note:

  • Only use the correct document types. You can send photos, Word documents or PDF files.  (File types ending in .jpg, .jpeg, .png, .doc, .docx).  If you find that you cannot upload a document, you can take a clear photo and send us that instead.
  • If you upload a document in error, you can remove it by selecting the 'Remove' button that displays below it.  You cannot remove an attached document once you have submitted the form.
  • You can attach more than one document, but you must upload each one separately.
  • For each document that you attach, please wait while it uploads.  The page will refresh, and you'll see the document listed on the screen under the 'Attachment' heading.

Submit form

When you have finished the form, select 'Submit'. You will receive an email confirming the form has been sent.

Once you have submitted the form you can find it in the Submitted Forms section of your account page. You can reach this by selecting your name at the top of the screen and choosing from the dropdown list.

From here you can download it or print it. It will remain visible in your account for 90 days.
