Completing an Adult Social Care Online form as a professional
Which forms are available?
This guidance is for professionals working with someone who has social care and support needs. If you're not a professional, please refer to Completing an Adult Social Care Online form.
If you want to make a referral that does not involve safeguarding, complete our Professional Referral form.
If you want to report a concern about an adult, you can use our safeguarding concern form.
If you're a health professional and you want to make a referral for reablement, you can use our Norfolk First Support form.
You can use our Disability Related Expenses form if you're working with someone that needs to complete this process.
You can make a referral to our Preparing for Adult Life team using our PfAL Referral form.
DOLS forms available online
You can use Adult Social Care Online to complete and submit the following DOLS forms:
- Form 1: Standard and urgent authorisation request
- Form 2: Request for a further standard authorisation
- Form 3: BIA combined assessment
- Form 3b: Renewal
- Form 3: Report style BIA
- Form 4: Mental capacity, mental health, and eligibility
- Form 7: Suspension of authorisation
- Form 10: Review request, notice of review, and outcome of review
