Moving on from the Life Beyond Care service

Feeling ready to move from the Life Beyond Care service?

If you are aged 21-24

We have a duty to support you until you reach 25-years-old. However when you turn 21, you can choose if and when you would like support from us.

What happens:

  • You will have a chat with your personal adviser (PA) and anyone else important to you. Together, you'll look at your goals and see if there's anything else we can help with. For example, advice on money, housing, education, work, relationships, or living independently.
  • Your PA will go through the independence checklist with you
  • Your PA will give you information about other support services in your local area
  • Your PA will give you a closure letter in the way you prefer - email or text. It will have information on how to get back in touch if you ever need support again, plus details about your local care leaver hubs.
  • Your PA will give you a thoughts and memories letter to remind you of your journey through Life Beyond Care and to recognise your achievements
  • Your PA will make sure you have a copy of our leaving well framework and ask your permission to contact you for feedback

Not every journey is smooth and that's okay. If you need support again, you can reach out to your old PA or email us at [email protected]Read our privacy notice to find out how we use your personal information.

Your PA can:

  • Help with specific tasks - without reopening your case
  • Signpost to other services
  • Meet-up to talk about restarting your support and updating your pathway plan

If possible, we'll try to reconnect you with your previous PA - if that's what you want.

If you are over 25

Once you're 25, we won't be able to reopen your case, but you can still get advice and guidance by emailing us at [email protected]Read our privacy notice to find out how we use your personal information.

However, before we close you to our service we will:

  • Have a final review of your pathway plan with your PA and anyone else important to you
  • Ensure you have received all of your financial entitlements
  • Go through the independence checklist with you
  • Give you information about other support services in your local area
  • Give you a thoughts and memories letter to remind you of your journey into Life Beyond Care and to recognise your achievements
  • Give you a closure letter with information on how to stay connected, access community support, and keep coming to our care leaver hubs. Even after you turn 25, you're always welcome!
  • Make sure you have a copy of our leaving well framework and ask your permission to contact you for feedback
