Placement plan

The placement plan will include details about:

  • Where you will live
  • What your foster carer must do for you
  • How often your social worker will visit you

The plan must be completed either before, or within five days of, you moving in with a new foster carer. It will describe your usual routines. For example, your bed time and when you get up in the morning. It will also include how your needs will be met in your foster placement.

The placement plan covers the following areas:

  • Arrangements for your education and training. This includes the name and address of your school, your designated teacher and details of any special educational needs
  • Your personal and cultural history
  • Your likes and dislikes
  • Arrangements for your health and dental care. This includes the name and address of your registered doctor and dentist
  • The rules of the placement
  • Agreements for family time between you and your family
  • How often the social worker will visit you and any dates for review meetings
  • If you have an independent visitor, the plan will include the arrangements for them to visit you and what their contact details are
  • The plan must show who can make decisions about:
    • Medical and dental treatment
    • Education and school trips
    • Overnight stays
    • Leisure and home life
    • Faith and religious observance
    • Use of social media
