What is in the plan

When we confirm your child is eligible for Short Breaks, we will discuss and agree their plan with you. We will base the plan on their identified needs and outcomes. An outcome is something your child wants to achieve by taking part in the activity. Every child is different, so your child's outcomes will be unique to them. 

The plan will include types of services, activities or items to meet these needs and outcomes.

Changing your Short Breaks plan

There is high demand for the Short Breaks service so we are unable to make changes to your child's Short Breaks plan, unless there is a significant change in your child's needs that means they are unable to access their chosen activities and/or services.
