This form should be completed to give information to the local authority (Norfolk County Council) about a pre-school child who has or is likely to have special educational needs under section 23 of the Children and Families Act 2014.
The County Council uses this information to decide if any additional services or support are needed for the child now or when they start pre-school or school. Sometimes they decide to carry out an Education, Health and Care needs assessment so that they can gather more information to help them make this decision.
The information also helps the Council to check that they have the services in place to support the needs of all children.
As part of Sections 23 and 24 of the Children and Families Act 2014, Health and education professionals will share information about the named child with Norfolk County Council. The local authority will respond to the notification, and act upon the shared information through the Norfolk County Council Early identification of Need (EIN) SEND Panel. We will use the information shared on this form to gather information to help us decide whether to carry out an Education Health Care (EHC) needs assessment.
It is not necessary for you to provide consent for us to do this, it is part of the County Council and health care organisations' legal responsibilities to share information and ensure that there is cooperation between all professionals involved in your child’s care.
