About the Outreach service

The crisis outreach team:

  • Provides immediate support to families and young people in crisis
  • Are available 24 hours a day, 7 days a week
  • Offers a range of services to de-escalate urgent situations
  • Engage in longer-term interventions, when necessary

Their goal is to help families stay together and prevent children from going into care.

Children with disabilities crisis outreach

The children with disabilities (CWD) crisis outreach support team:

  • Offers county-wide support to families and young people who are in crisis
  • Are registered with the Care Quality Commission (CQC)
  • Can provide domiciliary support

The Springwood Hub also offers planned and targeted support to prevent family breakdown. This is by assisting children and families within their own homes.
